Services

Skilled Nursing
Skilled nursing services can be provided by a Registered Professional Nurse in your home when prescribed by your physician.  Our skilled nursing services may include but are not limited to the following:
  • Initial assessment and evaluations

  • Pre- and post-operative care

  • Skilled nursing assessment with obtaining and monitoring vitals

  • Catheter care / change

  • Diabetic care

  • Infusion therapies

  • Lab draws / injections / IV therapies

  • Wound care / dressing change

  • Wound vac competent care

  • Tube feeding, G-tube care, and teaching

  • Orthopedics

  • Pulmonary care

  • Medication management and administration

  • Central line and port  maintenance

  • Other services as needed


Licensed Therapies
CCB HealthCare System offers a comprehensive range of quality clinical services including physical therapy, occupational therapy, speech and language pathology, and medical social services.

Physical Therapy

Our staff provides services with a focus on prevention, diagnosis, and treatment of movement dysfunction as well as enhancement of physical health and functional abilities.  Plans of care may include:

  • Mobility training to develop functional and safe mobility skills

  • Therapeutic exercise to increase strength, range of motion, and endurance

  • Balance facilitation to address safety and fall prevention

  • Modalities to address pain management

  • Home exercise programs and home safety evaluations

Occupational Therapy

Our staff helps patients develop skills needed to remain in their own homes and communities as well as the ability to engage in everyday activities. Plans of care may include:

  • Activities of Daily Living training (eating, grooming, dressing, etc.)

  • Instrumental Activities of Daily Living (management of personal or household affairs, etc.)

  • Therapeutic activities to increase range of motion, coordination, and endurance

  • Sensory integration

  • Facilitation of oral motor shills

Speech – Language Pathology

CCB HealthCare System staff members provide treatment to enable patients to communicate effectively and to improve oral motor skills to ensure safe swallowing.  Plans of care may include:

  • Communication – Auditory comprehension, expression, writing/reading skills, and speech intelligibility

  • Cognition – Memory and executive functioning

  • Swallowing – Diagnosis and treatment of swallowing problems and diet progression



Certified Home Health Aides

  • Light housekeeping

  • Personal hygiene needs (bath, shave, shampoo)

  • Laundry

  • Light meal preparation


Medical Social Services
HealthCare System provides social services for our patients including but not limited to:

  • Psychosocial support strategies and techniques for independent living

  • Family counseling

  • Community contact, education, and resource information
